The set-up: Photographer Adam and oncologist Dr. Gordon are chained in a bathroom with a dead body on the floor between them. Gordon quickly realizes that they are being tested by a serial killer named “Jigsaw."
The twist: The body in the middle of the floor isn't dead, it's actually Jigsaw himself, John Kramer - a former patient of Dr. Gordon's.

The set-up: Detective Matthews and his team catch John Kramer, but Kramer's computer monitors reveal a trap involving eight people, including Matthews's son, Daniel, and the first Saw's Amanda Young. Kramer ensures Matthews that Daniel will survive as long as Matthews follows his rules, but Matthews takes matters into his own hands.
The twist: The live feed on the computer monitors wasn't live - the trap had happened a few days earlier, and despite what it looked like on the screen, Daniel was safe the whole time.
Because Matthews didn't listen to Kramer, he's put into his own trap - by Jigsaw-survivor-turned-accomplice, Amanda Young.

'Saw 3D: The Final Chapter' - Dr. Gordon Is Jigsaw's Apprentice

The set-up: Dr. Gordon survived his test from the original Saw and now attends the Jigsaw Survivor Group. Meanwhile, Detective Hoffman seeks revenge against Jill, eventually killing her using a reverse bear trap.
The twist: Hoffman is kidnapped and chained up in the famous Saw bathroom by none other than Dr. Gordon himself. It's revealed he's been secretly helping Kramer with many of the surgery-filled traps throughout the series and was instructed to “act immediately” if anything happened to Jill.

The set-up: John Kramer's ex-wife Jill gives Detective Hoffman envelopes for future victims that Kramer had picked out before he died.
Meanwhile, health insurance executive William Easton is put in a series of tests while a mother and son watch from the sidelines.
The twist: The mother and son watching Easton aren't his wife and kid - they're the family of a man who died after Easton denied him medical coverage. It's now up to them to choose if he lives or dies. The son decides to pull a lever that drops acid-filled syringes onto Easton, killing him.
It's also revealed Jill hid one of Kramer's envelopes from Hoffman - one with Hoffman's picture. She places a reverse bear trap on his head, but he escapes it just in time.

The set-up: Amanda Young kidnaps Dr. Lynn Denlon and forces her to keep John Kramer alive while a man locked in a Jigsaw trap must confront the people responsible for his son's death. When Denlon saves Kramer's life by performing brain surgery, Kramer instructs Young to release Denlon from her trap. Young refuses and shoots Denlon instead.
The twist: Jeff enters the room just as Young shoots Denlon, and it's revealed that Jeff and Denlon are married. Jeff shoots Young, then kills Kramer with a power saw.

The set-up: Five people wake up in a Jigsaw trap and are told to go against their instincts in order to survive. As they participate in a series of tests, they quickly turn against each other, and die off one by one.
Meanwhile, FBI agent Strahm escapes the room Hoffman locked him in at the end of Saw IV and soon suspects Hoffman might be Jigsaw's apprentice. Strahm finds a tape that tells him to get inside a glass box, but when Hoffman appears in the room, Strahm stops the tape short and pushes the detective in instead.
The twist: Only two of the five people survive to face the last of the tests, where they realize no one had to die in any of the previous traps if they had all listened to the tape and worked together.
The box Hoffman is sealed inside lowers into the floor, while the walls to the room Strahm is standing in begin to close. The end of Strahm's tape states that if he didn't enter the box, he would be framed as Jigsaw's apprentice. Strahm is then crushed to death.
